Montelukast is a selective, orally active leukotriene receptor antagonist that blocks the cysteinyl leukotriene receptor (CysLT1). Cysteinyl leukotrienes (LTC4, LTD4, LTE4), products of arachidonic acid metabolism, are released by cells such as mast cells and eosinophils. These leukotrienes are involved in the pathophysiology of asthma and allergic rhinitis, contributing to airway swelling, smooth muscle contraction, and other inflammatory processes that lead to asthma symptoms.

No dose adjustment is needed for patients with mild-to-moderate hepatic or renal insufficiency or the elderly.
Trilock is compatible with common asthma treatments like theophylline, prednisone, oral contraceptives, digoxin, and warfarin without significant drug interactions. However, potent cytochrome P450 enzyme inducers like phenobarbital may reduce Trilock's effectiveness, so monitoring is advised.
Trilock should not be used in patients who are allergic to any of its components.
Montelukast can cross the placenta in animals, but there are no sufficient studies in pregnant women. It should be used during pregnancy only if necessary. Since the drug is excreted in breast milk, caution should be exercised when breastfeeding.
